Please fill out the New Patient Paperwork, all Co-pays are expected at the time of service. Please arrive 10-15 minutes prior to your appointment time.
Payment PolicyA 24-hour notice for cancellation or rescheduling is required. A no show fee of $40.00 will be assessed for all appointments where no notice has been received. If you are more than 10 minutes late for your appointment, you will be rescheduled. Additionally, if your paperwork is not complete upon arrival, and it requires more than 10 minutes to complete, your appointment will be rescheduled.
Please arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
